25. ASSESSMENT OF ROOT CANAL TREATMENT IN MANDIBULAR SECOND MOLAR USING XP FINISHER IRRIGATE ACTIVATION AND WARM VERTICAL COMPACTION, WITH COPI INDEX EVALUATION

Abstract

Objective: This study aimed to evaluate the clinical and radiographic outcomes of root canal treatment using XP Finisher File with irrigate activation and warm vertical compaction technique, assessed with the COPI index.


Subjects and Methods: In this clinical trial, 35 mandibular second molars of 33 pulpal and apical periodontitis patients were treated with nonsurgical endodontic procedures. The canals were shaped with Wave One Gold and irrigated with XP Shaper files, and warm vertical compaction was performed. The root canal treatment and healing process were monitored using CBCT with ETTI and COPI indices.


Results: The clinical evaluation conducted after 72 hours showed that 88.75% of the treated teeth were asymptomatic. 11.25% of the teeth were sensitive to percussion related to severe apical periodontitis (S3R3D3) and a score of L3 in the ETTI Index. The radiographic results from CBCT showed that 82.86% of the cases were classified as “healed,” 8.57% as “healing,” and 8.57% as “nonhealed.” Most cases improved the COPI index, with 29 cases classified as S0D0R0 and 6 cases as S1D1R1.


Conclusion: When treated with activated irrigation using XP Finisher File and warm vertical compaction technique, mandibular second molars with complex root canal anatomy showed successful outcomes. The COPI and ETTI indices were valuable in evaluating the results of root canal treatment and assessing periapical healing.
