OUTCOMES OF LAPAROSCOPIC CHOLECYSTECTOMY IN THE TREATMENT OF ACUTE CALCULOUS CHOLECYSTITIS IN ELDERLY PATIENTS AT HAI PHONG UNIVERSITY OF MEDICINE AND PHARMACY HOSPITAL

Abstract
Introduction: Acute calculous cholecystitis in the elderly often has a complicated course due to multiple comorbidities, increasing surgical risk. Laparoscopic cholecystectomy is the gold standard but requires careful assessment in this population.
Patients and Methods: A retrospective descriptive study was conducted on 44 patients aged ≥60 years diagnosed with acute calculous cholecystitis and treated with laparoscopic cholecystectomy at Hai Phong University of Medicine and Pharmacy Hospital from January to December 2025. Clinical, paraclinical, intraoperative, and postoperative data were collected and analyzed.
Results: The mean age was 66.2 years; females accounted for 61.4%. Comorbidities were present in 81.8% of patients, with 79.5% having more than two comorbid conditions. The main symptom was right upper quadrant pain (68.2%); abdominal guarding was observed in 59.1%, and leukocytosis in 68.2%. The mean operative time was 66.7 minutes; retrograde cholecystectomy was performed in 88.6% of cases. The mean postoperative hospital stay was 3.1 days. Good outcomes were achieved in 72.7% of patients; 27.3% developed postoperative complications, including 9.1% systemic complications.
Conclusion: Laparoscopic cholecystectomy for acute calculous cholecystitis in elderly patients yielded favorable treatment outcomes, with an acceptable postoperative complication rate and satisfactory recovery in the majority of patients.
