7. CLINICAL CHARACTERISTICS AND TREATMENT OUTCOMES OF PERFORATED COLONIC DIVERTICULITIS AT NGUYEN TRI PHUONG HOSPITAL (2017-2024)

Abstract
Introduction: Perforated colonic diverticulitis is a serious complication of colonic diverticular disease. If not diagnosed and treated promptly, it can lead to severe peritonitis and even death.
Methods: Retrospective study with a cases series included patients who was diagnosed postoperatively with Perforated colonic diverticulitis at Nguyen Tri Phuong Hospital from January 2017 to January 2024.
Results: There were 68 patients, the mean age was 56.3 years, 2.9% of cases had no abdominal pain, 86.8% of cases had no palpable abdominal mass, 72.1% of cases were classified as IB according to WSES. 56 patients had positive bacterial cultures, accounting for 82.4% of the total; 12 patients had negative cultures, accounting for 17.6%. The bacteria identified included Enterococcus faecium in 12 patients (17.6%) and E. coli in 35 patients (51.5%); the median hospital stay was 8 days and there was no mortality within 30 days after surgery.
Conclusions: Perforated colonic diverticulitis is common in middle-aged people, the most common clinical symptoms are abdominal pain, fever, diarrhea and peritoneal signs, diagnosis was mainly based on contrast-enhanced abdominal - pelvic CT scan. Promptly identification and treatment help reduce the rate of iatrogenic complications, complications and prevent mortality.
