Tóm tắt

Mục tiêu: Đánh giá hiệu quả giảm đau sau phẫu thuật chi trên ở trẻ em bằng gây tê đám rối
thần kinh cánh tay đường trên đòn dưới hướng dẫn siêu âm.
Đối tượng - Phương pháp: 54 bệnh nhi phẫu thuật chi trên, đủ tiêu chuẩn lựa chọn được phân
bố ngẫu nhiên vào 2 nhóm: Gây mê có phối hợp gây tê đám rối thần kinh cánh tay đường trên đòn
dưới hướng dẫn siêu âm (nhóm I) hoặc gây mê đơn thuần (nhóm II).
Kết quả: Điểm an thần Ramsay sau mổ: Nhóm I 100% bệnh nhân đạt ≤ 3, nhóm II có 85,18%
đạt ≤ 3 và 14,82% bệnh nhân đạt 4 điểm (p<0,05); thời gian giảm đau trung bình ở nhóm I 7,13 ±
0,64 giờ; thời gian tỉnh trung bình sau mổ nhóm I 7,85 ± 2,35, nhóm II 13,70 ± 2,65, (p<0,05); số
lần dùng thêm giảm đau sau mổ nhóm I là 1,30 ± 0,47, nhóm II 3,26 ± 0,59, (p<0,05).
Kết luận: Hiệu quả giảm đau sau mổ tốt hơn gây mê đơn thuần với: điểm an thần Ramsay sau
mổ tốt hơn, thời gian tỉnh mổ ngắm hơn, thời gian giảm đau sau mổ kéo dài và số lần dùng thêm
giảm đau sau mổ ít hơn đáng kể so với các bệnh nhi chỉ được gây mê đơn thuần.
