5. THE RESULTS OF A HYPERTENSION SELF-MANAGEMENT ENHANCEMENT INTERVENTION AT SELECTED COMMUNE HEALTH STATIONS IN NINH BINH PROVINCE

Abstract
Objective: This study aimed to evaluate the effectiveness of a community-based intervention to improve self-management practices among patients with hypertension at the commune health station level.
Method: A quasi-experimental pre-post comparison study with a control group was conducted in 6 commune health stations in Ninh Binh province, Vietnam. Patients were selected using systematic random sampling from hypertension management lists. The intervention included one-on-one counseling, group-based health education using flipcharts, a self-management handbook, and a blood pressure monitoring diary.
Result: After the intervention, the intervention group showed significant improvements across all five components of hypertension self-management. Patients in the intervention group had a significantly higher mean self-management score than those in the control group, with an adjusted difference of 19.5 points (p < 0.001). Higher self-management scores were associated with higher education levels and prior hypertension management at health facilities.
Conclusion: The intervention model demonstrated both feasibility and effectiveness in enhancing hypertension self-management at the community level. Findings provide practical evidence to support the scale-up of similar interventions in primary health care settings.
