2. ĐỘNG HỌC PROCALCITONIN Ở BỆNH NHÂN VIÊM PHỔI DO VI KHUẨN

Mục tiêu: Procalcitonin (PCT) giúp giảm thời gian sử dụng kháng sinh trên các bệnh nhân bị nhiễm khuẩn, đặc biệt ở bệnh nhân nhiễm trùng huyết. Tuy nhiên, vai trò của PCT ở bệnh nhân viêm phổi do vi khuẩn đa kháng thuốc (P-MDR) chưa được nghiên cứu nhiều, đặc biệt là ở Việt Nam. Do đó, đề tài tiến hành nhằm mục tiêu xác định động học PCT ở bệnh nhân viêm phổi do vi khuẩn.

Phương pháp nghiên cứu: Trong nghiên cứu theo dõi dọc này, chúng tôi phân tích động học PCT ở 121 bệnh nhân có kết quả vi sinh và lâm sàng khẳng định viêm phổi do vi khuẩn từ 1/2023 đến 10/2023.

Kết quả: Có 65 bệnh nhân viêm phổi do vi khuẩn đa kháng thuốc và 56 bệnh nhân viêm phổi do vi khuẩn không đa kháng thuốc (P-nMDR). Năm tác nhân phổ biến nhất gây viêm phổi vi khuẩn là Klebsiella pneumoniae (32,23%), Acinetobacter baumannii (26,45%), Pseudomonas aeruginosa (22,31%), Staphylococcus aureus (8,26%) và Escherichia coli (5,79%). Nồng độ PCT trung bình ở nhóm P-MDR cao hơn ở nhóm P-nMDR. Thời gian thanh thải PCT ở nhóm P-MDR chậm 2,13 lần so với nhóm P-nMDR. Có 64,46% bệnh nhân chưa ngưng sử dụng kháng sinh tại thời điểm PCT về dưới 0,05 ng/ml.

Kết luận: Nồng độ PCT trung bình ở nhóm P-MDR cao hơn ở nhóm P-nMDR. Thời gian thanh thải PCT ở nhóm P-MDR chậm hơn so với nhóm P-nMDR. Cần tiến hành nghiên cứu thử nghiệm lâm sàng ngẫu nhiên có đối chứng tiếp nối đề tài này để theo dõi động học PCT trên các nhóm bệnh nhân như COPD, viêm phổi vi khuẩn trong năm 2024.
