31. KẾT QUẢ ĐIỀU TRỊ KHÁNG SINH KẾT HỢP DẪN LƯU ÁP XE GAN TẠI BỆNH VIỆN NGUYỄN TRI PHƯƠNG

Tóm tắt

Đặt vấn đề: Dẫn lưu áp xe gan qua da là phương pháp điều trị ban đầu được lựa chọn cho những ổ áp xe có kích thước lớn hơn 5 cm đã hóa lỏng âm từ lâu đã được áp dụng tại nhiều bệnh viện tại Việt Nam.

Mục tiêu: Đánh giá hiệu quả điều trị áp xe gan bằng kháng sinh kết hợp dẫn lưu dưới hướng dẫn siêu âm.

Phương pháp nghiên cứu: Nghiên cứu hồi cứu các bệnh nhân (BN) áp xe gan đã được điều trị kháng sinh vừa kết hợp dẫn lưu ổ áp xe dưới hướng dẫn siêu âm tại Bệnh viện Nguyễn Tri Phương từ tháng 01/2017 đến tháng 12/2022.

Kết quả: Có 61 BN được đưa vào nghiên cứu. Tỷ lệ nam/nữ là 1,65/1; trung bình 56,1 ± 12,3 tuổi, tiền sử đái tháo đường chiếm 32,8%; triệu chứng lâm sàng nổi bật là sốt (88,5%), đau hạ sườn phải (85,2%). Ổ áp xe gan nằm ở gan phải chiếm 73,4%, đa phần là đơn ổ (78,7%). Kháng sinh điều trị đầu tay là Cephalosporin thế hệ III kết hợp Metronidazol truyền tĩnh mạch, đa số nhạy với Cephalosporin thế hệ III (49,2%), 8,2% là vi khuẩn đa kháng tiết ESBL sử dụng Carbapenems. Tỷ lệ điều trị thành công cao (96,7%).

Kết luận: Điều trị áp xe gan bằng kháng sinh kết hợp dẫn lưu dưới hướng dẫn siêu âm là một biện pháp hiệu quả và an toàn. Hầu hết các trường hợp đều chỉ can thiệp thủ thuật 1 lần với tỷ lệ thành công cao.
