11. NGHIÊN CỨU MỘT SỐ YẾU TỐ LIÊN QUAN ĐẾN TỬ VONG Ở TRẺ SƠ SINH ĐẺ NON TẠI TRUNG TÂM NHI KHOA, BỆNH VIỆN BẠCH MAI

Tóm tắt

Mục tiêu: Phân tích một số yếu tố liên quan đến tử vong sơ sinh ở trẻ đẻ non tại Trung tâm Nhi khoa, Bệnh viện Bạch Mai.

Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả thực hiện 324 trẻ sơ sinh đẻ non nhập viện tại Trung tâm Nhi khoa, Bệnh viện Bạch Mai.

Kết quả: Tỷ lệ tử vong ở trẻ sơ sinh đẻ non là 12,7%. Các yếu tố liên quan đến tử vong ở trẻ sơ sinh đẻ non bao gồm: bệnh lý của mẹ, hạ nhiệt độ, giảm SpO2 <90, điểm Apgar <5, trẻ đến từ nông thôn, trẻ phải thở máy, dị tật bẩm sinh kèm theo và cân nặng thấp là những yếu tố liên quan đến tử vong và làm tăng nguy cơ liên quan đến tử vong sơ sinh với mức ý nghĩa thống kê p<0,05. Năm yếu tố có ảnh hưởng rõ nhất đến tử vong ở trẻ sơ sinh đẻ non là hạ nhiệt độ, dị tật bẩm sinh, điểm Appgar tại thởi điểm 1 phút, thở máy và yếu tố địa dư.

Kết luận: Cần tăng cường công tác chăm sóc cho các bà mẹ khi mang thai đặc biệt mẹ có bệnh nền và sống ở nông thôn. Cần phát huy mô hình kết hợp sản-nhi nhằm hồi sức tích cực và kịp thời cho trẻ sơ sinh có nhiều nguy cơ sức khỏe.
