KẾT QUẢ THEO DÕI 6 THÁNG SAU PHẪU THUẬT PHẪU THUẬT CHẺ DỌC CÀNH LÊN HAI BÊN TRÊN MÔ XƯƠNG Ở BỆNH NHÂN SAI HÌNH XƯƠNG HẠNG III

Mục tiêu: Mô tả các thay đổi cấu trúc xương hàm dưới sau phẫu thuật chẻ dọc cành lên hai bên (bilateral sagittal split osteotomy - BSSO) nhằm điều trị tình trạng nhô xương hàm dưới.

Đối tượng và phương pháp: Nghiên cứu được thực hiện trên 46 bệnh nhân được phẫu thuật BSSO lùi xương hàm dưới. Phim sọ nghiêng được thu thập và phân tích tại thời điểm trước phẫu thuật và sau 6 tháng.

Kết quả: Tái cấu trúc xương hàm dưới tại điểm lồi cầu (Condylion) và góc hàm (Gonion) nhìn chung diễn ra theo hướng ra trước và lên trên. Không ghi nhận mối tương quan giữa thay đổi vị trí tại Condylion và Gonion với mức độ di chuyển phẫu thuật hoặc tái phát tại điểm B và Pogonion. Ghi nhận mối tương quan thuận có ý nghĩa thống kê giữa dịch chuyển theo chiều trước - sau sau phẫu thuật tại Gonion (r = 0,62; p < 0,001).

Kết luận: Các thay đổi tái cấu trúcxương hàm dưới sau BSSO xảy ra chủ yếu ở vùng lồi cầu và vùng góc hàm, trong khi vùng cằm tương đối ổn định.
