THUỐC ỨC CHẾ BRUTON TYROSINE KINASE TRONG ĐIỀU TRỊ MÀY ĐAY MẠN TÍNH TỰ PHÁT: CƠ SỞ BỆNH SINH VÀ BẰNG CHỨNG LÂM SÀNG – TỔNG QUAN TƯỜNG THUẬT

Bối cảnh: Nhiều bệnh nhân mày đay mạn tính tự phát (CSU) không kiểm soát triệu chứng dù đã dùng kháng histamin H1 liều tối đa hoặc omalizumab, tạo nhu cầu về liệu pháp đường uống mới.

Mục tiêu: Tổng quan hệ thống hóa cơ chế bệnh sinh liên quan Bruton tyrosine kinase (BTK) và bằng chứng lâm sàng về hiệu quả, an toàn của thuốc ức chế BTK trong CSU.

Phương pháp: Tìm kiếm hệ thống trên PubMed/MEDLINE, Google Scholar và trang FDA (1/2018–6/2026), dùng từ khóa “Bruton tyrosine kinase”, “BTK inhibitor”, “chronic spontaneous urticaria” và tên các thuốc ức chế BTK; ưu tiên thử nghiệm ngẫu nhiên có đối chứng và hướng dẫn quốc tế.

Kết quả: BTK là kinase then chốt dưới thụ thể FcεRI trên tế bào mast và bạch cầu ái kiềm, gây thoái hạt và giải phóng histamin trong CSU. Remibrutinib, thuốc ức chế BTK đường uống đầu tiên được FDA phê duyệt (9/2025) dựa trên hai thử nghiệm pha III REMIX-1/REMIX-2, cải thiện rõ rệt UAS7, khởi phát nhanh, hiệu quả đến tuần 52. Fenebrutinib và rilzabrutinib cũng cho kết quả khả quan ở pha II.

Kết luận: Thuốc ức chế BTK, đại diện là remibrutinib, là lựa chọn điều trị đường uống mới cho CSU kháng trị kháng histamin, có tiềm năng bổ sung các liệu pháp tiêm hiện có. Cần thêm dữ liệu thực tế tại Việt Nam để xác định vị trí tối ưu của thuốc.
