GÁNH NẶNG ĐAU SAU PHẪU THUẬT SỚM VÀ THỰC HÀNH QUẢN LÝ ĐAU SAU PHẪU THUẬT THAY KHỚP HÁNG TOÀN PHẦN TẠI MỘT BỆNH VIỆN TUYẾN CUỐI Ở VIỆT NAM.

Mục tiêu: Mô tả gánh nặng đau sau phẫu thuật sớm, thực trạng quản lý đau và một số biến phản ánh thực hành quản lý đau sau thay khớp háng toàn phần tại một bệnh viện tuyến cuối.

Đối tượng và phương pháp: Nghiên cứu quan sát tiến cứu được thực hiện trên 117 người bệnh trưởng thành được thay khớp háng toàn phần một bên tại Bệnh viện Chợ Rẫy từ tháng 5 năm 2025 đến tháng 4 năm 2026. Điểm đau theo thang số 0–10 được ghi nhận khi nghỉ và khi thực hiện hoạt động khả thi về mặt lâm sàng tại các thời điểm 6, 24, 48 và 72 giờ sau phẫu thuật. Đau nặng sớm được định nghĩa là có ít nhất một điểm đau ≥7 trong 24 giờ đầu sau phẫu thuật.

Kết quả: Tuổi trung vị của người bệnh là 54 tuổi; nam giới chiếm 71,8%. Có đau lúc nhập viện ở 99,1% người bệnh và 80,3% đã sử dụng thuốc giảm đau hoặc opioid trước phẫu thuật. Đau nặng sớm ghi nhận ở 36,8% người bệnh. Tại thời điểm 72 giờ, điểm đau trung bình khi nghỉ là 4,38 ± 1,34 và khi hoạt động là 5,53 ± 1,26. Paracetamol được sử dụng ở 98,3%, opioid theo mã hóa dữ liệu nguồn ở 93,2%, tramadol ở 19,7% và NSAID ở 9,4%; không ghi nhận trường hợp phong bế thần kinh ngoại biên. Trong phân tích hồi quy đa biến mang tính thăm dò, BMI cao hơn, tiền sử cơ xương khớp và việc ghi nhận sử dụng tramadol có mối liên quan với đau nặng sớm.

Kết luận: Gánh nặng đau vẫn còn đáng kể trong 72 giờ đầu sau thay khớp háng toàn phần. Cần chuẩn hóa việc ghi nhận loại hoạt động đi kèm điểm đau, thông tin sử dụng thuốc và dữ liệu vận động.
