25. XÁC ĐỊNH ĐỘ NHẠY CẢM VỚI KHÁNG SINH ĐIỀU TRỊ VIÊM PHỔI DO PHẾ CẦU KHUẨN Ở TRẺ EM ĐIỀU TRỊ TẠI BỆNH VIỆN NHI TRUNG ƯƠNG 2020

Mục tiêu: Đề tài xác định độ nhạy cảm với kháng sinh điều trị viêm phổi do phế cầu khuẩn ở trẻ em
< 5 tuổi tại Bệnh viện Nhi Trung ương nhằm xác định kháng sinh có hiệu quả điề trị tốt nhất.
Phương pháp nghiên cứu: Đề tài được thiết kế bằng phương pháp nghiên cứu mô tả thực nghiệm
tại labo
Kết quả: Phế cầu khuẩn có tỷ lệ kháng cao > 95% với các kháng sinh nhóm macrolid, kháng 90%
với cotrimoxazol, kháng 95,3% với clindamycin. Phế cầu khuẩn đa kháng kháng sinh chiếm 64%. Tỷ
lệ phế cầu khuẩn nhạy cảm 43,9% với penicillin G (MIC90=4 µg/ml), nhạy cảm 57,4% với cefotaxim
(MIC90= 4 µg/ml), nhạy cảm 61,1% với ceftriaxon (MIC90= 4 µg/ml), nhạy cảm 95% với amoxicillin
(MIC90 = 2 µg/ml), nhạy cảm 100% với rifampicin, linezolid và vancomycin.
Kết luận: Phế cầu khuẩn đã kháng với nhiều họ kháng sinh penicillin, cephalosporin, sulfamid,
macrolid. Còn nhạy cảm với họ rifampin, glycopeptid, oxazolidinon, quinolon. Vì vậy, khi điều trị
viêm phổi do phế cầu cần làm kháng sinh đồ lựa chọn kháng sinh nhạy cảm nhất.
