KỸ NĂNG GIAO TIẾP VỚI NGƯỜI BỆNH ĐANG TỨC GIẬN TRONG THỰC HÀNH ĐIỀU DƯỠNG: TỔNG QUAN TƯỜNG THUẬT DỰA TRÊN BẰNG CHỨNG

Đặt vấn đề: Người bệnh tức giận/kích động là tình huống thường gặp, có nguy cơ gây xung đột và bạo lực. Điều dưỡng tiếp xúc trực tiếp nên kỹ năng giao tiếp giữ vai trò quan trọng trong giảm leo thang.

Mục tiêu: Tổng hợp bằng chứng về cơ chế tức giận, kỹ năng giao tiếp và hiệu quả đào tạo giảm leo thang trong thực hành điều dưỡng.

Đối tượng và phương pháp: Tổng quan tường thuật có phương pháp, định hướng bằng chứng, tìm kiếm trên PubMed, Scopus, Web of Science và CINAHL các nghiên cứu từ 01/2010 đến 01/03/2026 về người bệnh tức giận/kích động, giảm leo thang và đào tạo giao tiếp.

Kết quả: Giảm leo thang là quá trình đa thành phần gồm lắng nghe chủ động, đồng cảm, kiểm soát giọng nói, giao tiếp không lời, tự điều chỉnh cảm xúc và giải quyết vấn đề. Đào tạo có thể cải thiện kiến thức, thái độ và sự tự tin ngắn hạn; bằng chứng về giảm bạo lực thực tế chưa nhất quán. Một nghiên cứu cụm ngẫu nhiên trong khoa tâm thần cấp tính ghi nhận giảm gây hấn và giảm biện pháp cưỡng chế sau đào tạo.

Kết luận: Giao tiếp là yếu tố then chốt nhưng không phải giải pháp duy nhất. Cần đào tạo có cấu trúc, mô phỏng, phản hồi và hỗ trợ tổ chức để nâng cao an toàn chăm sóc.
