Mục tiêu: Tổng hợp các bằng chứng cập nhật về dịch tễ học, cơ chế bệnh sinh, đặc điểm lâm sàng, chẩn đoán, điều trị và chỉ định sử dụng lại thuốc miễn dịch ở bệnh nhân viêm phổi kẽ liên quan đến thuốc ức chế điểm kiểm soát miễn dịch.

Phương pháp: Thực hiện tổng quan tường thuật trên cơ sở tìm kiếm các nghiên cứu được công bố từ năm 2018-2026. Ba ca lâm sàng điều trị tại Bệnh viện K minh họa cho các tình huống thường gặp trong thực hành.

Kết quả: Viêm phổi kẽ liên quan đến thuốc ức chế điểm kiểm soát miễn dịch có biểu hiện lâm sàng và hình ảnh học rất đa dạng. Corticosteroid vẫn là nền tảng điều trị. Quyết định sử dụng lại thuốc ức chế điểm kiểm soát miễn dịch cần được cá thể hóa.

Kết luận: Mặc dù hiếm gặp, viêm phổi kẽ do thuốc ức chế điểm kiểm soát miễn dịch là một biến chứng làm thay đổi phác đồ điều trị, đòi hỏi phải có quy trình chẩn đoán và chiến lược điều trị phù hợp.
