ĐÁNH GIÁ KẾT QUẢ XẠ TRỊ BỔ TRỢ UNG THƯ VÚ GIAI ĐOẠN I-III TẠI BỆNH VIỆN UNG BƯỚU NGHỆ AN

Tóm tắt

Mục tiêu: Đánh giá tỷ lệ sống thêm không bệnh tái phát tại chỗ- tại vùng, tỷ lệ sống thêm không bệnh, tỷ lệ sống thêm toàn bộ bệnh nhân ung thư vú giai đoạn I-III xạ trị bổ trợ tại bệnh viện ung bướu Nghệ An.

Đối tượng và phương pháp: Nghiên cứu mô tả hồi cứu, có theo dõi dọc trên 91 bệnh nhân ung thư vú giai đoạn I-III, được xạ trị bổ trợ sau mổ tại Bệnh viện Ung bướu Nghệ An từ tháng 1/2020 đến hết tháng 12/2020.

Kết quả: Đối tượng nghiên cứu có tuổi trung bình là 51,66 ± 11,41 tuổi, tỷ lệ bệnh nhân giai đoạn I là 3,3%, giai đoạn II là 69,2%, giai đoạn III là 27,5%. Tỷ lệ tái phát tại chỗ, tại vùng là 4,4%, di căn xa là 15,4%, chưa tái phát di căn là 80,2%. Tỷ lệ sống thêm không bệnh tái phát tại chỗ, tại vùng (LRRFS), tỷ lệ sống thêm không bệnh (DFS),  tỷ lệ sống thêm toàn bộ (OS) 3 năm lần lượt là 96,4%, 77,8%, 84,5% và 5 năm lần lượt là 95%, 70,1%, 74,5%. Tỷ lệ và mức độ biến chứng muộn liên quan đến xạ trị thấp với phù tay là 5.49%, xơ phổi là 12,08% và xơ cứng thành ngực là 2,19%. Giai đoạn bệnh là yếu tố tiên lượng (HR = 0,36; CI 95%: 0,13–0,98; p = 0,046) liên quan đến thời gian sống thêm không bệnh.

Kết luận: Xạ trị bổ trợ là phương pháp điều trị hiệu quả và an toàn đối với bệnh nhân ung thư vú giai đoạn I–III, tỷ lệ biến chứng điều trị thấp. Giai đoạn bệnh là yếu tố tiên lượng đến thời gian sống thêm không bệnh.
