Tóm tắt

Mục tiêu: Đánh giá hiệu quả bước đầu và tính an toàn của hóa xạ trị tiền phẫu với kỹ thuật xạ trị điều biến liều theo thể tích hình cung ở bệnh nhân ung thư thực quản biểu mô vảy 1/3 giữa và 1/3 dưới giai đoạn II-IVA.

Phương pháp: Mô tả có theo dõi dọc trên 71 bệnh nhân được hóa xạ trị tiền phẫu phác đồ Paclitaxel-Carboplatin, kỹ thuật xạ trị điều biến liều theo thể tích hình cung.

Kết quả: Tuổi trung bình của bệnh nhân là 56,1; 100% bệnh nhân là nam giới; giai đoạn III chiếm 53,5%; tỷ lệ di căn hạch (N+) trước điều trị là 70,4%. Sau hóa xạ trị, tỷ lệ đáp ứng chung là 90,1%; tỷ lệ phẫu thuật được sau hóa xạ trị là 91,5%; tỷ lệ đáp ứng hoàn toàn trên mô bệnh học là 32,4%. Giai đoạn u và mức độ đáp ứng sau hóa xạ trị có mối tương quan tới khả năng đáp ứng hoàn toàn trên mô bệnh học.

Kết luận: Hóa xạ trị tiền phẫu bằng kỹ thuật xạ trị điều biến liều theo thể tích hình cung cho ung thư thực quản 1/3 giữa và 1/3 dưới biểu mô vảy là phác đồ hiệu quả, dung nạp tốt, mang lại tỷ lệ đáp ứng cao.
