Bệnh phenylxeton niệu (PKU) là rối loạn di truyền lặn trên nhiễm sắc thể số 12, đặc trưng bởi khiếm
khuyết enzyme phenylalanine hydroxylase (PAH) làm nồng độ phenylalanin (Phe) tăng cao trong
máu, gây độc hệ thần kinh và khuyết tật trí tuệ. Tỉ lệ hiện mắc PKU trên thế giới khoảng 1/10.000
- 20.000 trẻ sơ sinh, tuy nhiên tỉ lệ người lành mang gen bệnh PKU và phổ biến thể gây bệnh trên
người Việt Nam còn rất hạn hữu. Hiện nay, kĩ thuật giải trình tự thế hệ mới (NGS) cho phép thực hiện
giải trình tự hàng triệu phân tử ADN của nhiều bệnh nhân trong một phản ứng, nhờ vậy nâng cao hiệu
suất, năng suất , hiệu quả về chi phí và thời gian. Mục tiêu: Mô tả thực trạng người lành mang gen và
phổ biến thể gây bệnh PKU. Phương pháp: 2937 phụ nữ đến khám sức khỏe tại Trung tâm Di truyền
lâm sàng và Hệ gen - Bệnh viện Đại học Y Hà Nội và Viện Di truyền Y học được khảo sát biến thể
trên gen PAH bằng kĩ thuật NGS. Kết quả: Có 70 người mang biến thể gây bệnh (2.4%), trong đó có
12 loại biến thể gồm: R408Q (1/2937), V388M (2/2937), P314T (2/2937), E280K (1/2937), R241fs
(1/2937), R241C (1/2937), Y206* (1/2937), Y204C (1/2937), Q172H (56/2937), H170Q (2/2937),
R111* (1/2937), L41F (1/2937). Biến thể phổ biến nhất là Q172H, có 2 biến thể hiếm là L41F và
R241fs chưa được báo cáo về tần số alen trên ClinVar và Ensembl. Kết luận: Kết quả nghiên cứu
này tạo tiền đề cho các nhà hoạch định chính sách và các chuyên gia y tế đưa ra các chiến lược phù
hợp cho các chương trình chẩn đoán trước sinh, kế hoạch tư vấn di truyền.
