Đặt vấn đề: Động mạch thân tạng (Celiac Axis – CA) thường xuất phát từ động mạch chủ bụng tại mức đốt sống thắt lưng thứ nhất. Sự xuất phát của CA từ động mạch chủ ngực là một biến thể giải phẫu hiếm gặp. Việc phát hiện biến thể này có ý nghĩa quan trọng trong lập kế hoạch phẫu thuật và can thiệp mạch.

Ca lâm sàng: Bệnh nhân nam, 62 tuổi, được chụp CT đa dãy đầu dò vùng bụng – tiểu khung do thoát vị bẹn trái. Hình ảnh tình cờ phát hiện CA xuất phát từ động mạch chủ ngực, phía trên lỗ động mạch chủ cơ hoành, cao hơn vị trí điển hình khoảng 13 mm. Không ghi nhận chèn ép hay triệu chứng lâm sàng gợi ý thiếu máu tạng.

Kết luận: Nguồn gốc cao của động mạch thân tạng là biến thể hiếm gặp, cần phân biệt với hội chứng dây chằng cung giữa. Chụp CT đa dãy với tái tạo 3D giúp mô tả chính xác giải phẫu mạch máu, hỗ trợ quan trọng trong chẩn đoán và lập kế hoạch phẫu thuật.
