TỈ LỆ TRẦM CẢM VÀ YẾU TỐ LIÊN QUAN Ở NGƯỜI CAO TUỔI RỐI LOẠN CHỨC NĂNG TIỀN ĐÌNH

Mục tiêu: Xác định tỉ lệ trầm cảm và phân tích các yếu tố liên quan ở người cao tuổi rối loạn chức năng tiền đình đang điều trị nội trú tại Bệnh viện Bạch Mai.

Phương pháp: Nghiên cứu mô tả cắt ngang được thực hiện trên 84 người bệnh từ 60 tuổi trở lên mắc rối loạn chức năng tiền đình từ tháng 8/2024 đến tháng 3/2025. Tình trạng trầm cảm được sàng lọc và đánh giá qua thang điểm trầm cảm Hamilton. Số liệu được xử lý thống kê bằng phần mềm SPSS 25.0. 

Kết quả: Tỉ lệ người bệnh có biểu hiện trầm cảm là 29,8%, trong đó nhóm trầm cảm mức độ nặng chiếm tỉ trọng cao nhất (15,5%). Phân tích các yếu tố liên quan cho thấy các đặc điểm lâm sàng làm tăng nguy cơ mắc trầm cảm có ý nghĩa thống kê (p < 0,05) bao gồm: tiền sử mắc rối loạn tiền đình trước đây (p = 0,009), sự hiện diện của triệu chứng ù tai (p < 0,001), thời gian tồn tại triệu chứng chóng mặt kéo dài (p < 0,001), và tần suất xuất hiện từ 3 cơn chóng mặt trở lên mỗi tuần (p < 0,001).

Kết luận: Trầm cảm là một gánh nặng y tế lớn ở người bệnh tiền đình cao tuổi, liên quan chặt chẽ đến mức độ nghiêm trọng của các triệu chứng thể chất. Cần đánh giá sức khỏe tâm thần định kỳ khi người bệnh đến điều trị rối loạn chức năng tiền đình.
