ĐÁNH GIÁ KHẢ NĂNG SINH SẢN Ở NAM GIỚI VÔ TINH KHÔNG TẮC NGHẼN ĐỒNG THỜI MANG BẤT THƯỜNG DI TRUYỀN ĐƯỢC PHẪU THUẬT MICRO-TESE TẠI BỆNH VIỆN BƯU ĐIỆN

Mục tiêu: Đánh giá tỷ lệ và đặc điểm bất thường di truyền ở nam giới vô tinh không do tắc nghẽn (NOA), đồng thời phân tích kết quả thu tinh trùng và kết quả sinh sản sau mổ vi phẫu tinh hoàn tìm tinh trùng (micro-TESE) tại Bệnh viện Bưu điện.

Phương pháp: Nghiên cứu mô tả hồi cứu được thực hiện trên 309 bệnh nhân được chẩn đoán NOA và điều trị bằng kỹ thuật micro-TESE tại Trung tâm Hỗ trợ sinh sản, Bệnh viện Bưu điện từ tháng 9/2022 đến tháng 9/2024. Trong đó, 90 bệnh nhân có xác định bất thường di truyền và đầy đủ hồ sơ bệnh án được đưa vào phân tích. Các chỉ số được đánh giá bao gồm đặc điểm lâm sàng, kết quả xét nghiệm di truyền, tỷ lệ thu nhận tinh trùng bằng micro-TESE và kết quả sinh sản sau tiêm tinh trùng vào bào tương noãn (ICSI).

Kết quả: Tỷ lệ bất thường di truyền ở bệnh nhân NOA là 29,1%. Các bất thường nhiễm sắc thể giới tính chiếm 96,7% trường hợp. Hội chứng Klinefelter là bất thường thường gặp nhất (70%), tiếp theo là vi mất đoạn AZFc trên nhiễm sắc thể Y (22,3%). Tỷ lệ thu nhận tinh trùng chung từ micro-TESE đạt 54,4%. Tỷ lệ thu nhận tinh trùng ở nhóm hội chứng Klinefelter và nhóm vi mất đoạn AZFc lần lượt là 54% và 55%. Trong số các trường hợp thu nhận được tinh trùng thành công, tỷ lệ trẻ sinh sống cộng dồn sau ICSI đạt 61,2%.

Kết luận: Bất thường di truyền chiếm tỷ lệ cao ở nam giới mắc NOA, chủ yếu liên quan đến các bất thường nhiễm sắc thể giới tính. Kỹ thuật micro-TESE cho thấy hiệu quả khả quan trong thu nhận tinh trùng và hỗ trợ sinh sản ở nhóm bệnh nhân này, đặc biệt ở các trường hợp hội chứng Klinefelter và vi mất đoạn AZFc. Việc kết hợp micro-TESE và ICSI đóng vai trò quan trọng trong điều trị vô sinh nam có bất thường di truyền.
