Mục tiêu: Tổng quan các mô hình dự báo nguy cơ ung thư phổi trên thế giới nhằm mô tả đặc điểm, yếu tố nguy cơ, hiệu năng dự báo và khả năng ứng dụng, từ đó đề xuất định hướng cho Việt Nam. Phương pháp: Nghiên cứu tổng quan hệ thống được thực hiện trên các cơ sở dữ liệu trong và ngoài nước đến tháng 01/2025. Các nghiên cứu được lựa chọn theo tiêu chí PICOS và quy trình PRISMA. Các chỉ số đánh giá mô hình bao gồm độ nhạy, độ đặc hiệu và diện tích dưới đường cong ROC (AUC). Kết quả: Tổng cộng 72 nghiên cứu được đưa vào phân tích. Các mô hình dự báo chủ yếu sử dụng hồi quy logistic hoặc hồi quy Cox. Các yếu tố nguy cơ thường gặp gồm tuổi, giới, tình trạng và mức độ hút thuốc, chỉ số BMI, tiền sử gia đình ung thư phổi và bệnh phổi mạn tính. Các mô hình trên người hút thuốc có hiệu năng dự báo trung bình đến tốt (AUC 0,77–0,88), trong khi các mô hình cho người không hút thuốc còn hạn chế nhưng đang được quan tâm nhiều hơn tại châu Á. Kết luận: Nhiều mô hình dự báo nguy cơ ung thư phổi đã được phát triển, tuy nhiên, phần lớn dựa trên quần thể phương Tây. Việc áp dụng tại Việt Nam cần được kiểm định và hiệu chỉnh phù hợp với đặc điểm dịch tễ trong nước.
