KẾT QUẢ BƠM CEMENT SINH HỌC CÓ BÓNG (BALLOON KYPHOPLASTY) ĐIỀU TRỊ XẸP THÂN ĐỐT SỐNG DO LOÃNG XƯƠNG TẠI BỆNH VIỆN ĐA KHOA TỈNH SƠN LA

Tóm tắt

Mục tiêu: Mô tả đặc điểm bệnh nhân và đánh giá hiệu quả giảm đau; cải thiện chức năng và thay đổi X-quang sau balloon kyphoplasty ở xẹp thân đốt sống do loãng xương (OVCF).

Đối tượng và phương pháp: Loạt ca theo dõi dọc trước–sau gồm 56 bệnh nhân điều trị tại Bệnh viện Đa khoa tỉnh Sơn La (01/2024–03/2026). Ghi nhận VAS và Roland–Morris trước mổ, sau mổ ngày 1, tháng 1 và tháng 3. Chỉ số X-quang gồm góc xẹp, góc Cobb, chiều cao thân đốt (tường trước/giữa/sau). So sánh trước–sau bằng t-test ghép đôi (p<0,05).

Kết quả: Tuổi trung bình 69,18±9,33; nữ 67,9%. VAS giảm từ 7,61±1,12 xuống 2,73±1,14 (ngày 1), 1,63±0,84 (tháng 1) và 1,32±0,86 (tháng 3) (đều p<0,001). Roland–Morris giảm từ 17,43±2,20 xuống 10,39±1,37; 8,75±1,05; 7,82±0,69 (đều p<0,001). Góc xẹp cải thiện từ 13,26±1,95° xuống 9,79±1,45°; góc Cobb từ 17,97±2,16° xuống 12,71±1,87°; chiều cao tường trước/giữa tăng có ý nghĩa (đều p<0,001). 82,1% không biến chứng; rò xi măng bờ trước 10,7% và rò vào đĩa đệm 7,1%, tất cả không triệu chứng và không cần can thiệp.

Kết luận: Balloon kyphoplasty giúp giảm đau nhanh và bền vững, cải thiện chức năng và chỉnh hình X-quang rõ rệt ở OVCF, với tỷ lệ rò xi măng không triệu chứng thấp, khẳng định hiệu quả và an toàn khi triển khai tại tuyến tỉnh.
