NHÂN MỘT TRƯỜNG HỢP THỦNG THỰC QUẢN ĐẾN MUỘN BIẾN CHỨNG RÒ THỰC QUẢN-TRUNG THẤT

Đặt vấn đề: Thủng thực quản là một tình trạng có thể đe dọa tính mạng với tỷ lệ tử vong dao động từ 10-40%. Việc chẩn đoán muộn, đặc biệt là các trường hợp do hóc xương cá, làm tăng nguy cơ viêm trung thất và rò thực quản - trung thất mạn tính.

Ca bệnh: Bệnh nhân nam 28 tuổi bị đau sau xương ức nghi sau nuốt xương cá và tự điều trị. Ban đầu triệu chứng giảm dần, nhưng 5 tháng sau, bệnh nhân xuất hiện ho kéo dài. CT scan ngực phát hiện rò thực quản-trung thất mạn. Bệnh nhân được điều trị bằng đặt stent thực quản. Sau 8 tuần, nội soi kiểm tra cho thấy vị trí rò đã lành, stent đã di chuyển xuống dạ dày và được lấy bỏ an toàn.

Kết luận: Thủng thực quản đến muộn có thể diễn tiến âm thầm và dễ bị bỏ sót. Các triệu chứng hô hấp dai dẳng ở bệnh nhân có tiền sử nuốt dị vật cần được khảo sát hình ảnh để loại trừ rò mạn tính. Đặt stent thực quản là một lựa chọn điều trị hiệu quả.
