GIẢM MẪN CẢM THÀNH CÔNG VỚI D-PENICILLAMINE Ở CA BỆNH DỊ ỨNG KHI ĐIỀU TRỊ BỆNH WILSON

Đặt vấn đề: D-Penicillamine là thuốc chelat thường dùng trong điều trị bệnh Wilson, nhưng có thể gây phản ứng quá mẫn muộn, thường xuất hiện sau vài tuần điều trị.

Ca lâm sàng: Bệnh nhi nữ 14 tuổi được chẩn đoán bệnh Wilson xuất hiện ban dạng sởi lan tỏa kèm ngứa sau 7 ngày dùng D-Penicillamine. Test áp bì thực hiện sau 4 tuần cho kết quả dương tính, gợi ý phản ứng quá mẫn muộn. Do hạn chế về khả năng tiếp cận thuốc thay thế như trientine, bệnh nhi được lựa chọn giảm mẫn cảm chậm với D-Penicillamine theo phác đồ kinh nghiệm, kết hợp corticosteroid tiền điều trị và kháng histamin khi cần. Trong quá trình này, trẻ ghi nhận một đợt ban nhẹ, được kiểm soát tốt. Sau 40 ngày, bệnh nhi dung nạp liều điều trị 900 mg/ngày và tiếp tục điều trị ngoại trú.

Kết luận: Trường hợp này cho thấy test áp bì có thể hỗ trợ chẩn đoán quá mẫn muộn với D-Penicillamine; đồng thời, giảm mẫn cảm chậm là một lựa chọn khả thi trong bối cảnh hạn chế thuốc thay thế, tuy nhiên cần thêm nghiên cứu để đánh giá tính an toàn và hiệu quả.
