THU TINH TRÙNG BẰNG PHƯƠNG PHÁP MICRO TESE Ở BỆNH NHÂN VÔ TINH KHÔNG DO TẮC VÀ MỘT SỐ YẾU TỐ LIÊN QUAN

Đặt vấn đề: Vi phẫu thuật thu tinh trùng từ tinh hoàn (microdissection testicular spem
extraction - micro tese) là phương pháp thu tinh trùng hiệu quả cao, ít nguy cơ gây tổn
thương tinh hoàn thứ phát. Mục tiêu: Đánh giá tỷ lệ thu tinh trùng bằng phương pháp micro
tese ở bệnh nhân (BN) vô tinh không do tắc (nonobstructive azoospermia - NOA) và một số
yếu tố liên quan. Đối tượng và phương pháp nghiên cứu: 132 BN NOA được làm micro tese
tại Viện Mô phôi Lâm sàng Quân đội - Học viện Quân y từ 5/2017 - 12/2019. Kết quả: Tỷ
lệ thu tinh trùng là 40,15%. Không có mối liên quan giữa tuổi, BMI, thời gian vô sinh, AZF
với tỷ lệ thu tinh trùng. Có mối liên quan giữa thể tích tinh hoàn, nồng độ nội tiết, mô bệnh
học với tỷ lệ thu tinh trùng. Điểm cắt của thể tích tinh hoàn phải và trái là 5,5mL tương ứng
với độ nhạy là 86,3% và 84,3%, độ đặc hiệu là 50,6% và 51,9%, diện tích đường cong là
0,707 và 0,676 (p < 0,001 và p < 0,01). Điểm cắt của FSH, LH và testosteron tương ứng là
13,41mIU/mL, 7,64mIU/mL và 3,02ng/mL với độ nhạy 74,4%; 69,9%; 86,8%, độ đặc hiệu
58,5%; 56,6%; 46,8% và diện tích đường cong tương ứng là 0,666; 0,625; 0,647. Kết luận:
Mặc dù tìm thấy mối liên quan giữa thể tích tinh hoàn, nồng độ FSH, LH, testosteron và mô
bệnh học với tỷ lệ thu tinh trùng bằng phương pháp micro tese ở BN NOA nhưng giá trị tiên
lượng là thấp.
