Tóm tắt

Đặt vấn đề: Hiện nay, 50% các trường hợp hiếm muộn có liên quan đến nam giới. Đã có
nhiều nghiên cứu về mối liên quan giữa tuổi và chỉ số khối cơ thể (BMI) với chất lượng tinh
trùng của nam giới, tuy nhiên các kết quả ghi nhận còn nhiều tranh cãi. Tại Việt Nam hiện
nay vẫn chưa có nhiều nghiên cứu về vấn đề này. Nghiên cứu của chúng tôi thực hiện nhằm
tìm hiểu mối liên quan giữa tuổi và BMI với chất lượng tinh trùng của nam giới Việt Nam.
Mục tiêu: Đánh giá mối liên quan giữa tuổi và chỉ số khối cơ thể (BMI) với chất lượng tinh
trùng của nam giới Việt Nam.
Đối tượng: 1144 nam giới đến khám tại khoa Nam học & Y học giới tính, Bệnh viện Đại
học Y Hà Nội, từ tháng 1/2017 đến hết tháng 12/2021.
Phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang.
Kết quả: Nhóm trên 45 tuổi có chỉ số thể tích tinh dịch, tổng số lượng tinh trùng, tỉ lệ tinh
trùng hình thái bình thường thấp hơn đáng kể so với hai nhóm 16-30 và 31-45 tuổi. Nhóm
nam giới có BMI≥25 có chỉ số thể tích tinh dịch và tổng số tinh trùng thấp hơn có ý nghĩa
thống kê so với các nhóm có BMI<18,5 và BMI từ 18,5 đến 24,99.
Kết luận: Tuổi cao ảnh hưởng tiêu cực đến hầu hết các khía cạnh đánh giá chất lượng tinh
trùng trên tinh dịch đồ: làm giảm thể tích tinh dịch, giảm tổng số lượng tinh trùng, giảm tỉ lệ
tinh trùng hình thái bình thường, tỉ lệ tinh trùng sống và tỉ lệ tinh trùng di động tiến tới nhanh.
Tình trạng thừa cân béo phì gây có liên quan với tình trạng giảm thể tích tinh dịch cũng như
giảm tổng số tinh trùng ở nam giới Việt Nam. Mối liên quan giữa tình trạng thiếu cân và chất
lượng tinh trùng là chưa rõ ràng.
