ĐÁNH GIÁ KẾT QUẢ BẢO VỆ TẾ BÀO NỘI MÔ GIÁC MẠC CỦA NHẦY CHỨA MANITOL TRONG PHẪU THUẬT PHACO TRÊN NGƯỜI BỆNH CÓ MẬT ĐỘ TẾ BÀO NỘI MÔ THẤP

Tóm tắt

Mục tiêu: Đánh giá tính an toàn và hiệu quả bảo vệ tế bào nội mô giác mạc của chất nhầy nội nhãn chứa manitol trong phẫu thuật Phaco điều trị đục thủy tinh thể ở người bệnh có mật độ tế bào nội mô thấp.

Đối tượng và phương pháp: Nghiên cứu tiến cứu, có nhóm chứng, thực hiện trên các người bệnh đục thủy tinh thể có chỉ định phẫu thuật Phaco với mật độ tế bào nội mô từ 1000-2000 tế bào/mm² tại Bệnh viện Mắt Sài Gòn Vinh từ tháng 10/2024 đến tháng 5/2025. Người bệnh được phân ngẫu nhiên vào 3 nhóm: nhóm A sử dụng nhầy chứa sodium hyaluronate 2% kết hợp manitol 0,5%; nhóm B sử dụng nhầy chứa sodium hyaluronate 1,4%; và nhóm C sử dụng nhầy chứa hydroxypropyl methylcellulose 2,4%. Các chỉ số mật độ và hình thái tế bào nội mô, độ dày giác mạc trung tâm và thị lực chỉnh kính tối đa được đánh giá trước phẫu thuật và sau phẫu thuật 1 tuần, 1 tháng và 3 tháng.

Kết quả: Tại thời điểm 1 tuần sau phẫu thuật, nhóm A có mật độ tế bào nội mô cao hơn, hệ số biến thiên thấp hơn, tỷ lệ tế bào lục giác cao hơn và độ dày giác mạc trung tâm thấp hơn có ý nghĩa thống kê so với hai nhóm còn lại (p < 0,05). Thị lực sau phẫu thuật ở nhóm A cải thiện tốt hơn rõ rệt. Đến thời điểm 1 tháng sau phẫu thuật, phù giác mạc hầu như đã hồi phục hoàn toàn ở tất cả các nhóm và sự khác biệt về thị lực không còn ý nghĩa thống kê.

Kết luận: Chất nhầy nội nhãn chứa manitol 0,5% giúp bảo vệ tế bào nội mô giác mạc tốt hơn trong phẫu thuật Phaco ở người bệnh có mật độ tế bào nội mô thấp, góp phần giảm phù giác mạc và thúc đẩy hồi phục thị lực sớm sau phẫu thuật.
