Mục tiêu: Đánh giá kết quả điều trị phẫu thuật chấn thương tinh hoàn tại Bệnh viện Hữu
nghị Việt Đức từ tháng 1/2017 đến tháng 6/2022.
Đối tượng và phương pháp: Nghiên cứu mô tả theo dõi hồi cứu, chọn mẫu thuận tiện trên
38 bệnh nhân điều trị phẫu thuật chấn thương tinh hoàn tại Trung tâm Nam học, Bệnh viện
Hữu nghị Việt Đức từ tháng 1/2017 đến tháng 6/2022.
Kết quả: Độ tuổi trung bình của bệnh nhân chấn thương tinh hoàn là 27,84 ± 10,71 tuổi (từ
10-53 tuổi) và 63,2% bệnh nhân đã kết hôn. Nguyên nhân thường gặp nhất là do tai nạn giao
thông chiếm 52,6%. Thời gian nằm viện trung bình là 15,26 ± 17,14 ngày (từ 6-32 ngày). Có
22 bệnh nhân (57,8%) tổn thương tinh hoàn phải và 1 trường hợp tổn thương tinh hoàn 2 bên.
Thời gian phẫu thuật trung bình là 65,44 ±25,15 phút (45-100 phút). Có 34 trường hợp bảo
tồn được tinh hoàn và 4 trường hợp phải cắt bỏ tinh hoàn dập nát. Hậu phẫu không có trường
hợp nào chảy máu, hoại tử tinh hoàn, có 2 trường hợp nhiễm trùng vết mổ. Thời gian theo
dõi sau mổ trung bình là 30 tháng (2-57 tháng). Có 19/38 số bệnh nhân có giảm kích thước
tinh hoàn bên chấn thương, 7/25 bệnh nhân giảm nồng độ testosterone, 9/16 bệnh nhân tinh
trùng thấp dưới mức trung bình. Không có trường hợp nào đau kéo dài sau mổ.
Kết luận: Chấn thương tinh hoàn là một cấp cứu tiết niệu – nam học chủ yếu xảy ra ở người
trẻ tuổi, việc chẩn đoán sớm và can thiệp hợp lý sẽ tăng tỉ lệ bảo tồn tinh hoàn cũng như các
chức năng nội tiết, sinh sản.
