HIỆU QUẢ ỨNG DỤNG THANG ĐIỂM BLATCHFORD TRONG TIÊN LƯỢNG BỆNH NHÂN XUẤT HUYẾT TIÊU HÓA CAO DO LOÉT DẠ DÀY-TÁ TRÀNG TẠI KHOA CẤP CỨU, BỆNH VIỆN ĐA KHOA ĐỨC GIANG NĂM 2025

Tóm tắt

Mục tiêu: Mô tả đặc điểm lâm sàng và cận lâm sàng của bệnh nhân xuất huyết tiêu hóa cao và đánh giá giá trị tiên lượng của thang điểm Blatchford trong dự đoán nhu cầu can thiệp y khoa.

Phương pháp: Nghiên cứu cắt ngang, hồi cứu trên 53 bệnh nhân xuất huyết tiêu hóa cao do loét dạ dày-tá tràng nhập viện tại Khoa Cấp cứu, Bệnh viện Đa khoa Đức Giang từ tháng 3-8 năm 2025.

Kết quả: Tuổi trung bình là 66,8 tuổi với 83% là nam giới. Điểm Blatchford ≥ 6 chiếm 90,6% bệnh nhân. Can thiệp y khoa được thực hiện ở 73,6% trường hợp. Tại điểm cắt ≥ 6, thang điểm cho thấy độ nhạy xuất sắc (100%), giá trị dự báo âm (100%) và độ chính xác (92,5%). Blatchford ≥ 6 điểm (OR = 15,8, p = 0,003) và phân loại Forrest nguy cơ cao IA-IIB (OR = 8,5, p = 0,007) là hai yếu tố tiên lượng độc lập của nhu cầu can thiệp y khoa.

Kết luận: Thang điểm Blatchford là công cụ có giá trị trong phân tầng nguy cơ sớm tại khoa cấp cứu, giúp xác định chính xác nhóm bệnh nhân nguy cơ thấp có thể không cần can thiệp cấp cứu.
