CHẤT LƯỢNG PHỤC HỒI SAU PHẪU THUẬT VÀ MỘT SỐ YẾU TỐ LIÊN QUAN Ở BỆNH NHI TẠI BỆNH VIỆN SẢN NHI NGHỆ AN NĂM 2025

Mục tiêu: Đánh giá chất lượng phục hồi sau phẫu thuật bằng bộ công cụ PostopQRS và phân tích một số yếu tố liên quan ở bệnh nhi tại Bệnh viện Sản Nhi Nghệ An năm 2025.

Đối tượng và phương pháp: Nghiên cứu mô tả tiến cứu có phân tích 384 bệnh nhi (6-15 tuổi) được phẫu thuật chương trình tại Bệnh viện Sản Nhi Nghệ An từ ngày 01/12/2024-30/6/2025. Dữ liệu thu thập từ hồ sơ bệnh án và phỏng vấn bằng bộ công cụ PostopQRS tại các thời điểm 15 phút, 40 phút, ngày 1 và ngày 3 hậu phẫu. Xử lý và phân tích số liệu bằng SPSS 22.0.

Kết quả: Nhóm tuổi 6-10 tuổi chiếm ưu thế (82,3%; Tuổi trung bình: 8,3 ± 2,3). 62,2% trẻ là nam. Tỷ lệ suy dinh dưỡng và suy dinh dưỡng nặng: 9,1% và 11,2%. Vô cảm nhiều nhất là gây mê nội khí quản (78,6%), tiếp theo gây mê mask thanh quản (20,3%), gây mê tĩnh mạch (1,1%). Ở thời điểm 40 phút sau phẫu thuật, tỷ lệ phục hồi: sinh lý 99%; đau 65,1%; cảm xúc 97,7%; nhận thức 7,8%. Phục hồi hoạt động hàng ngày 18,5% (ngày 1) và 23,7% (ngày 3). Phục hồi toàn bộ tại ngày 1 và ngày 3 lần lượt là 5,2% và 22,4%. Không ghi nhận biến chứng nghiêm trọng sau mổ. Các yếu tố liên quan đến phục hồi đau sớm gồm nhóm tuổi, phương pháp phẫu thuật và mức độ lo âu tiền phẫu.

Kết luận: Thang đo PostopQRS cho phép đánh giá toàn diện quá trình phục hồi sau phẫu thuật ở trẻ em. Chất lượng phục hồi tốt có mối liên quan với độ tuổi, loại phẫu thuật và yếu tố tâm lý tiền phẫu. Cần có các chương trình can thiệp giáo dục tiền phẫu phù hợp lứa tuổi.
