ĐẶC ĐIỂM LÂM SÀNG VÀ CẬN LÂM SÀNG CỦA VIÊM PHỔI DO PHẾ CẦU Ở TRẺ EM TẠI BỆNH VIỆN NHI TRUNG ƯƠNG

Tóm tắt

Mục tiêu: Viêm phổi do phế cầu là nguyên nhân gây tử vong hàng đầu ở trẻ em tại các nước đang
phát triển. Bệnh viện Nhi Trung ương thường tiếp nhận những trường hợp viêm phổi nặng do phế
cầu. Nghiên cứu tiến hành nhằm xác định đặc điểm lâm sàng và cận lâm sàng của viêm phổi do phế
cầu trên các bệnh nhi tại Bệnh viện Nhi Trung ương.
Đối tượng và phương pháp: Nghiên cứu tiến cứu mô tả loạt ca bệnh trên 169 bệnh nhi viêm phổi
do phế cầu tuổi từ 1 tháng đến 15 tuổi điều trị tại Bệnh viện Nhi Trung ương từ tháng 9 năm 2015
đến tháng 12 năm 2018.
Kết quả: Viêm phổi do phế cầu chiếm 41,52% viêm phổi ở trẻ em, tỷ lệ nam/nữ là 2/1, tập trung chủ
yếu ở độ tuổi từ 2 tháng đến 2 tuổi (78,1%). Các triệu chứng lâm sàng đa dạng không đặc hiệu. Sốt
chiếm 89,94% trong đó sốt cao chiếm 42,01%, ho (94,67%), chảy mũi (88,76%), khò khè 78,11%,
tím tái (12,43%), ran ẩm (80,47%), rút lõm lồng ngực (62,7%). Bạch cầu tăng chiếm 91,72%, CRP
tăng (68,05%), thiếu máu (40,24%). Hình ảnh X-quang viêm phế quản phổi chiếm 79,29%, viêm
phổi thùy (17,16%), tràn dịch màng phổi (5,33%). Viêm phổi nặng do phế cầu chiếm 68%.
Kết luận: Phế cầu hiện đang là nguyên nhân hàng đầu gây viêm phổi ở trẻ em. Triệu chứng lâm sàng
của viêm phổi do phế cầu ở trẻ em không điển hình. Tổn thương trên X-quang chủ yếu là hình ảnh
viêm phế quản phổi khác với các mô tả trước đây đa số là viêm phổi thùy. Vì tỷ lệ viêm phổi nặng
do phế cầu cao nên cần mở rộng các phương pháp chẩn đoán phế cầu giúp phát hiện sớm, không bỏ
sót và điều trị đúng nhằm hạn chế diễn biến nặng và biến chứng của bệnh.
