49. GIÁ TRỊ TIÊN LƯỢNG CỦA MỘT SỐ CHỈ SỐ SIÊU ÂM TIM, PHỔI VÀ CƠ HOÀNH CHO CAI THỞ MÁY Ở BỆNH NHÂN HỒI SỨC NGOẠI KHOA

Tóm tắt

Mục tiêu: Nghiên cứu nhằm đánh giá độ chính xác của siêu âm phổi, tim và cơ hoành trong việc dự đoán thất bại cai thở máy ở bệnh nhân hồi sức ngoại khoa.

Phương pháp: Bệnh nhân được thở máy qua ống nội khí quản trên 48 giờ và sẵn sàng thực hiện thử nghiệm thở tự nhiên lần đầu tiên. Thử nghiệm thở tự nhiên trong 1 giờ bằng chế độ hỗ trợ áp lực PS 7 cmH2O. Siêu âm ngay trước khi bắt đầu thử nghiệm thở tự nhiên.

Kết quả: 22/69 bệnh nhân (31,9%) thất bại cai thở máy. Nhóm thất bại có điểm siêu âm phổi tổng, vùng trước, vùng trước-ngoài và vùng sau cao hơn, di động cơ hoành và thay đổi độ dày cơ hoành theo hô hấp thấp hơn có ý nghĩa thống kê so với nhóm thành công. Phân tích hồi quy đa biến thấy điểm siêu âm phổi trước-ngoài > 9,5, di động cơ hoành < 1,3 cm và độ dày cơ hoành theo hô hấp < 17,7% trước thử nghiệm thở tự nhiên là các yếu tố dự báo độc lập của thất bại cai thở máy.

Kết luận: Siêu âm phổi và cơ hoành có giá trị dự đoán thất bại cai thở máy tốt ở bệnh nhân hồi sức ngoại khoa.
