Tóm tắt

Mục tiêu: Đánh giá hiệu quả của can thiệp y tế từ xa (thông qua ứng dụng YouMed) trong kiểm soát HbA1c và glucose huyết tương ở người bệnh đái tháo đường typ 2 ngoại trú tại Trung tâm Y tế quận Liên Chiểu, thành phố Đà Nẵng, thông qua phân tích dọc bằng mô hình hồi quy.

Phương pháp: Thiết kế nghiên cứu phỏng can thiệp so sánh trước sau có nhóm chứng. Theo dõi 230 người bệnh (nhóm can thiệp và nhóm chứng với tỷ lệ 1:1) tại 3 thời điểm: trước can thiệp, sau 3 tháng và sau 6 tháng. Sử dụng mô hình hồi quy logistic để phân tích xác suất đạt ngưỡng HbA1c < 7% và glucose < 7,2 mmol/L sau điều chỉnh các yếu tố gây nhiễu.

Kết quả: Nhóm can thiệp có khả năng đạt HbA1c < 7% cao hơn nhóm chứng gần 2 lần (OR = 1,998; KTC 95%: 1,171-3,408; p = 0,011). Không có khác biệt có ý nghĩa thống kê giữa hai nhóm về khả năng đạt glucose mục tiêu (p = 0,318).

Kết luận: Can thiệp y tế từ xa có hiệu quả trong cải thiện kiểm soát HbA1c, là hướng triển vọng trong quản lý bệnh mạn tính tại tuyến y tế cơ sở.
