15. ĐÁNH GIÁ HIỆU QUẢ BƠM VANCOMYCIN TRONG XƯƠNG DỰ PHÒNG TRONG PHẪU THUẬT THAY KHỚP GỐI

Đặt vấn đề: Phương pháp bơm vancomycin vào trong xương trước mổ trong phẫu thuật thay khớp gối cho thấy tăng đáng kể nồng độ vancomycin tại chỗ, ít tác dụng phụ toàn thân, giúp phòng ngừa nhiễm trùng sau mổ, đặc biệt với tụ cầu kháng methiciline (MRSA).

Đối tượng và phương pháp nghiên cứu: Nghiên cứu hồi cứu 1279 bệnh nhân phẫu thuật thay toàn bộ khớp gối tại bệnh viện đa khoa Tâm Anh HCM (01/2022- 05/2024) với thời gian theo dõi ít nhất 90 ngày. 795 bệnh nhân không bơm vancomycin, 484 bệnh nhân được bơm 500mg vancomycin pha 120ml NaCl 0.9% vào đầu gần xương chày sau khi garo, trước khi rạch da. Tất cả các bệnh nhân đều được sử dụng cephazolin dự phòng trước mổ và phác đồ sau mổ giống nhau.

Kết quả: Có 11 trường hợp nhiễm trùng ở nhóm không bơm vancomycin (1.38%), 01 trường hợp nhiễm trùng ở nhóm có bơm vancomycin (0.2%, p=0.037). Không ghi nhận biến chứng toàn thân ở nhóm bơm vancomycin, nồng độ vancomycin ghi nhận ở đầu và cuối cuộc mổ tại chỗ đều cao.

Kết luận: Bơm vancomycin trong xương trong phẫu thuật thay khớp gối bước đầu cho thấy hiệu quả phòng ngừa nhiễm trùng sau mổ, dễ thực hiện và không tác dụng phụ toàn thân.
