19. NGHIÊN CỨU HIỆU QUẢ CỦA SUFENTANIL VÀ FENTANYL TRONG GÂY MÊ PHẪU THUẬT CẮT AMIDAN Ở TRẺ EM

Đặt vấn đề: Cắt amidan là một trong những phẫu thuật phổ biến nhất ở trẻ em, được thực hiện dưới gây mê nội khí quản và có mức độ đau nhiều sau phẫu thuật. Các thuốc giảm đau trong phẫu thuật không chỉ có hiệu quả giảm đau trong phẫu thuật mà còn ảnh hưởng đến giảm đau sau phẫu thuật. Mục tiêu của nghiên cứu này là đánh giá hiệu quả của Sufentanil so với Fentanyl để gây mê phẫu thuật cắt amidan ở trẻ em.

Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang trên 119 bệnh nhân từ 2-16 tuổi có tình trạng sức khỏe theo phân độ của Hội Gây mê Hoa Kỳ I hoặc II, có chỉ định mổ cắt amidan, được khởi mê ngẫu nhiên với 0,3 µg/kg Sufentanil hoặc 2 µg/kg Fentanyl. Đánh giá chất lượng đặt nội khí quản dựa trên sự thay đổi mạch, huyết áp trước và sau đặt nội khí quản, thang điểm đánh giá chất lượng đặt nội khí quản, mức độ kích động và cường độ đau sau phẫu thuật được đánh giá tại các thời điểm 2, 4, 6 giờ và 24 giờ sau phẫu thuật, và ghi nhận các tác dụng không mong muốn.

Kết quả: Chất lượng đặt nội khí quản tốt và xuất sắc ở nhóm Sufentanil và Fentanyl lần lượt là 100% và 92,6% (p < 0,05). Tỷ lệ bệnh nhân kích động sau gây mê ở nhóm Sufentanil là 23,1% và ở nhóm Fentanyl là 40,7% (p < 0,05). Tỷ lệ bệnh nhân có tình trạng kích động nặng sau gây mê ở nhóm Sufentanil và Fentanyl lần lượt là 3,1% và 18,5% (p < 0,05). Điểm FPS-R tại các thời điểm 2, 4, 6 giờ sau phẫu thuật và điểm đau PPPM-SF tại thời điểm 24 giờ sau phẫu thuật ở nhóm Sufentanil thấp hơn nhóm Fentanyl, sự khác biệt này có ý nghĩa thống kê (p < 0,05).

Kết luận: Sufentanil hoặc Fentanyl bảo đảm hiệu quả giảm đau trong gây mê phẫu thuật cắt amidan ở trẻ em. Sufentanil cho thấy chất lượng đặt nội khí quản tốt hơn, tỷ lệ kích động sau phẫu thuật ít hơn và góp phần giúp giảm đau sau phẫu thuật tốt hơn gây mê với Fentanyl.
