Những bệnh nhân sống sót sau khi ngừng tuần hoàn hay ngừng tim thường bị rối loạn chức năng thần kinh nghiêm trọng vì tổn thương não do thiếu oxy và chết tế bào não do tái tưới máu. Theo hướng dẫn của Hiệp hội Tim mạch Hoa Kỳ, liệu pháp hạ thân nhiệt mục tiêu đã trở thành một liệu pháp tiêu chuẩn để bảo vệ não sau khi tái lập tuần hoàn tự nhiên trở lại thành công ở những bệnh nhân bị ngừng tim ngoài bệnh viện. Đây là báo cáo trường hợp của một bệnh nhân nữ 21 tuổi bị ngừng tim ngoài bệnh viện và được hồi sinh sau thời gian hồi sức tim phổi kéo dài. Liệu pháp hạ thân nhiệt mục tiêu sau đó được làm mát bên ngoài bằng chăn trong 24 giờ để bảo vệ não. Bệnh nhân đã được cai máy thở dần dần và thành công mà không bị tổn thương thần kinh. Ngày càng có nhiều bằng chứng về liệu pháp hạ thân nhiệt mục tiêu và cơ chế bảo vệ não của nó ở những bệnh nhân có loạn nhịp gây ngừng tim không thể sốc điện, đặc biệt chú trọng đến các kết quả về thần kinh. Bài viết này nhấn mạnh vai trò của liệu pháp hạ thân nhiệt mục tiêu trong mọi hồi sức tim phổi thành công bất kể nhịp tim.
