17. ĐẶC ĐIỂM LÂM SÀNG, CẬN LÂM SÀNG VÀ KẾT QUẢ ĐIỀU TRỊ BỆNH COVID-19 Ở PHỤ NỮ CÓ THAI TẠI BỆNH VIỆN BỆNH NHIỆT ĐỚI TRUNG ƯƠNG

Tóm tắt

Mục tiêu: Mô tả đặc điểm lâm sàng, cận lâm sàng và kết quả điều trị bệnh COVID-19 ở phụ nữ có thai (PNCT) điều trị tại Bệnh viện Bệnh Nhiệt đới Trung ương năm 2020-2022.

Đối tượng và phương pháp nghiên cứu: Nghiên cứu hồi cứu trên hồ sơ bệnh án của 209 PNCT mắc COVID-19 có triệu chứng lâm sàng được chẩn đoán xác định COVID-19 bằng xét nghiệm RTPCR-SARS-CoV-2, nhập viện điều trị Bệnh viện Bệnh Nhiệt đới Trung ương từ 01/01/2020 đến 30/06/2022.

Kết quả: Các triệu chứng thường gặp của bệnh COVID-19 ở PNCT là: ho (82,3%) và sốt (67,5%). Đặc điểm cận lâm sàng bất thường của bệnh COVID-19 ở PNCT là: thiếu máu (34,5%), giảm bạch cầu lympho (51,7%), tăng D-dimer (94,7%), tăng fibrinogen (59,7%), tăng CRP (78%), tăng LDH (40,3%). Kết quả điều trị của PNCT: tỷ lệ tử vong là 2,9%. Kết quả của thai kỳ: 35,4% (74/209) PNCT chấm dứt thai kỳ, trong đó 85,1% (63/74) sinh thai non tháng.

Kết luận: Triệu chứng hay gặp nhất của bệnh COVID-19 ở PNCT là ho và sốt. Bệnh COVID-19 ở PNCT có xu hướng tăng D-dimer, fibrinogen, CRP, LDH và giảm bạch cầu lympho, giảm hemoglobin. PNCT mắc COVID-19 có tỷ lệ bị bệnh nặng cao nhưng tỷ lệ tử vong không cao. PNCT mắc COVID-19 có xu hướng sinh non cao hơn.
