Abstract

Objective: Splenic rupture is a common injury in blunt abdominal trauma, which often appear in multiple trauma. The disease is usually severe with high mortality risk.

Aim: Study the results of splenectomy to treat splenic rupture due to trauma.

Subjects and methods: A prospective and cross-sectional study on 58 of blunt splenic patients undergoing splenectomy at Military Hospital 103 from January 2019 to June 2023.

Results: Studied 58 patients with splenic rupture underwent splenectomy, emergency surgical indication was 41.4%; Grade III, Grade IV, grade V of splenic injury was 3.4%; 77.6%; 18.9%. Pancreas was the most combined injured organ with 25.9% while left kidney rupture was 3.4% and jejunal rupture was 1.7%. Postoperative complications encountered 20.68%, of which pancreatic fistula, residual abscess, deep vein thrombosis was 13.8%; 1.7%; 1.7%. The good postoperative outcome was 86.2%, the average was 8.6%, 3 patients had severe development and death was 5.2%. There were significant relationship between treatment results, surgical indications and combined intra-abdominal injuries with p < 0.05.

Conclusion: Immediate emergency and emergency splenectomy were 41.4% and 58.6%, good postoperative results was 86.2%. Blunt abdominal trauma with splenic rupture often damages the combined organs such as pancreas, kidney and jejunum, requiring early diagnosis and prompt surgery to reduce the risk of complications.
