Objectives: To systematically review evidence on the cost-effectiveness of abemaciclib in the treatment of HR+/HER2− advanced or metastatic breast cancer.
Research methods: A systematic review was conducted in accordance with PRISMA 2020. PubMed, Cochrane, and Embase were searched up to March 1, 2026. Eligible studies were assessed using the CHEERS checklist. Cost data were converted to Vietnamese dong (VND) at 2025 values.
Results: Eight studies were included, conducted in the Americas and Asia, mainly from the payer or healthcare system perspective. Compared with endocrine therapy alone, abemaciclib was cost-effective in 4 of 7 relevant studies. When compared with other CDK4/6 inhibitors, the findings were less consistent. The ICER was mainly influenced by drug costs, utility values, and clinical effectiveness parameters.
Conclusion: The cost-effectiveness of abemaciclib was inconsistent across studies and depended on the comparator, treatment line, drug price, and reimbursement context. Further research in Vietnam is needed to support reimbursement decisions and price negotiation.
