Objectives: To determine the TSH target achievement rate after the first course of levothyroxine and associate factors with target achievement in papillary thyroid cancer (PTC) patients post-total thyroidectomy at the National Hospital of Endocrinology. Methods: A cross-sectional observational study was conducted on 194 patients (aged 18-64) with T1-T2N0M0 PTC. Evaluations were performed for 6-12 weeks following the initial levothyroxine prescription. TSH targets were individualized based on ATA 2025 clinical response categories including excellent/indeterminate; biochemical incomplete, structural incomplete. Results: The TSH target achievement rate was 34.5%, 46.4% TSH below target range, while 19.1% were above the target. The mean levothyroxine dose was 1.81 ± 0.24 mcg/kg/day. Associated factors with targeted TSH after initial treatment included age, sex, BMI and levothyroxin dose/kg/day. Conclusions: The rate of achieving TSH targets after the first treatment course was 34.5%. Age, sex, BMI, and levothyroxine dose/kg/day were possible associated factors with targeted TSH after first treament course.
