Objective: To evaluate the diagnostic value of immature granulocyte (IG) parameters in early onset neonatal infections (EONI) in preterm neonates.
Methods: A cross-sectional analytical study was conducted on 114 preterm neonates aged 0–72 hours, with all eligible neonates included at the Neonatal Unit, Danang Family Hospital, between August 1, 2024, and June 1, 2026; data were collected retrospectively and prospectively. EONI was diagnosed according to the National Guidelines on Neonatal Care and Treatment issued by the Ministry of Health (2025). Neonates without EONI were followed until 72 hours of age and were not diagnosed with EONI.
Results: There were 41 neonates with EONI and 73 without EONI. The median IG# and IG% were higher in the EONI group than in the non-EONI group (p < 0.001). IG# and IG% yielded AUCs of 0.723 and 0.709, respectively (p < 0.001). The optimal cut-off value for IG# was 0.18 G/L, with a sensitivity of 65.9% and specificity of 72.6%, while that for IG% was 1.85%, with a sensitivity of 51.2% and specificity of 82.2%
Conclusions: Both IG# and IG% demonstrated moderate discriminative ability for EONI and may support the diagnosis of EONI in preterm neonates.
