Background: Impacted or malpositioned mandibular third molars often require surgical extraction, a procedure commonly associated with postoperative complications such as facial swelling, pain, and trismus. Prednisolone, a potent corticosteroid, has been proposed to mitigate these sequelae; however, the clinical efficacy of a single preoperative oral dose still warrants further evaluation under real-world clinical conditions in Vietnam. Objective: To evaluate the efficacy of a single 20mg oral dose of Prednisolone administered preoperatively for mandibular third molar extraction surgery at the dental clinic of Pham Ngoc Thach University of Medicine (2025-2026). Subjects and Methods: A randomized, placebo-controlled, single-blind clinical trial was conducted on 64 ASA I patients indicated for extraction of impacted or malpositioned mandibular third molars. All patients received prophylactic Amoxicillin 2g orally 60 minutes prior to surgery, in accordance with American Dental Association (ADA) guidelines. The intervention group (n=32) additionally received 20mg oral Prednisolone, while the control group (n=32) received a matching placebo. Patients with contraindications to corticosteroids or penicillin, or at high risk of infection, were excluded. Pain intensity was assessed using the Numeric Rating Scale (NRS, 0-10), facial swelling was determined by the sum of anatomical landmark distances, and maximal mouth opening (mm) was measured using calipers. Categorical variables were compared using the Chi-square test or Fisher's exact test; continuous variables were compared using the paired-samples t-test and independent-samples t-test (or Mann-Whitney U test), with statistical significance set at p<0.05. Results: GEE models revealed significant Group x Time interaction effects across all swelling parameters (AC, AD, BE) and mouth opening (p<0,001). The Prednisolone group exhibited significantly lower NRS pain scores on days 3 and 7 (p<0,001). On postoperative day 3, the Prednisolone group demonstrated significantly reduced facial swelling across all three lines: AC (MD=+0.638 cm, p=0.043), AD (MD= +0.691 cm, p=0.033), and BE (MD=+0.866 cm, p=0.006). By day 7, significant swelling reduction persisted for line AC (MD=+0.706 cm, p=0.007) and line BE (MD=+0.781 cm, p=0.015). Furthermore, the Prednisolone group achieved significantly superior recovery of mouth opening on day 7 (5.14 ±0.81 cm vs. 4,46 ±0,88 cm; MD=-0.678 cm, p= 0.016). Conclusion: A single 20mg preoperative oral dose of Prednisolone safely and effectively reduces postoperative pain, controls facial edema, and promotes recovery of mouth opening following mandibular third molar extraction.
