Objective: To evaluate the outcomes of endoscopic Draf IIB frontal sinus surgery using the outside-in approach.
Materials and Methods: A prospective, descriptive case series of 25 patients who underwent endoscopic Draf IIB via the outside-in approach at Ho Chi Minh City ENT Hospital from January 2022 to December 2025.
Results: Mean age 49.4 ± 14.1 years; 56% male; 56% with prior surgery. Pathologies: recurrent sinusitis 40%, osteoma 32%, inverted papilloma 16%, mucocele 12%. Operative time 169.4 ± 64.4 minutes; blood loss median 200 [150–300] ml. No major intraoperative complications. Total SNOT-22 decreased from median 40 [34–44] to 3 [2–4] at 6 months (p < 0.001). Wide-open patency: 88% (22/25) at 6 months; partial stenosis 12%; no obstruction. CT at ≥ 6 months postoperative: 100% patent on CT. Median follow-up was 16.4 months. No complications or recurrence were noted.
Conclusion: These preliminary results suggest that Draf IIB via the outside-in approach is feasible, safe, and effective for unilateral frontal sinus disease, achieving high rates of frontal neo-ostium patency and significant improvement in patient-reported outcomes over the available follow-up period. Studies with larger cohorts and longer follow-up are needed to confirm these findings.
