Objective: To determine the prevalence of sarcopenia using the SARC-F, SARC-CalF, Ishii equation; and to evaluate the level of agreement among these tools in outpatients with type 2 diabetes mellitus.
Methods: A descriptive cross-sectional study was conducted on 240 outpatients with type 2 diabetes mellitus from December 2024 to February 2025 at two hospitals in Hung Yen province. Sarcopenia screening was performed using SARC-F, SARC-CalF, and the Ishii equation, and the level of agreement was assessed using the Kappa coefficient.
Results: The detection rates of sarcopenia were 25.8% by SARC-F, 38.7% by SARC-CalF, and 72.9% by the Ishii equation (in the subgroup aged ≥ 65 years). A significantly higher risk was observed in patients aged ≥ 65 years and those on insulin therapy for ≥ 5 years (p < 0.05). The Kappa coefficient between SARC-F and SARC-CalF was 0.448; within the ≥ 65 age group, the Kappa coefficients ranged from 0.206 to 0.377.
Conclusions: The detection rate of sarcopenia in type 2 diabetes patients differs significantly across the SARC-F, SARC-CalF, and Ishii formula. Sarcopenia risk is elevated in older age groups and with prolonged disease duration. Given the low-to-moderate agreement among these tools, implementing routine screening and combining multiple assessment methods are crucial for early diagnosis and prompt intervention.
