EVALUATE THE SURGICAL OUTCOMES OF TRANSORAL ENDOSCOPIC THYROIDECTOMY (TOETVA) TREATED MALE THYROID CANCER PATIENTS

Abstract

Objectives: This study is aimed at evaluating the effectiveness of the transoral endoscopic thyroidectomy (TOETVA) in the treatment of thyroid cancer in male patients.

Patients and methods: A prospective study was conducted on 51 male thyroid cancer patients treated with TOETVA from January 2019 to June 2024 at Hanoi Medical University Hospital.

Results: The mean patient age was 31.5 (17-56 years old), with 88.2% having solitary tumors, mostly in the right lobe (54.9%). Papillary thyroid carcinoma (94.1%) was the predominant result. The mean tumor size was 8.4 mm. Lobectomies comprised 96.1% of surgeries. The average operative time was 84.9 (60-150) min, and blood loss was minimal 6.7 (5-15) ml. Postoperative complications included transient hoarseness (3.9%) and mouth-opening difficulty (1.96%), with no permanent severe hypocalcemia or hoarseness. No cases of completion thyroidectomy or conversion to open surgery due to TOETVA failure were recorded. On average, 5 (0-16) lymph nodes were harvested, with a median of 2 (0-10) malignant nodes. Most tumors were T1 (96.1%), and pN1a was observed in 43.1% of the cases. Patient satisfaction with cosmetic results was high.

Conclusions: TOETVA is a safe and effective treatment for male patients with early-stage thyroid cancer, offering minimal complications and excellent cosmetic results.
