Objective: To describe the characteristics of published studies and methods for detecting biofilms in patients with chronic rhinosinusitis (CRS).
Literature search and synthesis methods: A scoping review was conducted following the Arksey–O'Malley five-step framework and reported according to PRISMA-ScR guidelines. Literature was searched in PubMed, the Cochrane Library, Google Scholar, and domestic medical journals/libraries for publications in English or Vietnamese between January 2010 and June 2026. Three independent reviewers screened studies based on the PCC framework (Population: CRS patients; Concept: direct tissue-based biofilm detection methods; Context: no restrictions). Data were extracted and descriptively summarized; methodological quality assessment (not required for scoping reviews per JBI recommendations) and quantitative analysis were not performed.
Results: From an initial 784 records, after removing duplicates and screening, 61 full-text articles were assessed and 22 studies were included (39 were excluded at the full-text stage). The studies were published between 2010 and 2023 (81.8% between 2010 and 2014), featured small sample sizes (63.6% <50), and utilized tissue/mucosal specimens collected during surgical procedures (22/22). Direct detection methods included CLSM/SDCM (50.0%), BacLight LIVE/DEAD (31.8%), SEM (27.3%), H&E staining (27.3%), Gram staining (22.7%), FISH (18.2%), and PNA-FISH, immunofluorescence, and OCT (4.5% each). Two additional studies evaluated in vitro biofilm formation capabilities and provided further supporting evidence. Positive rates ranged from 54.2% to 88.0% in the disease group (reaching 97.1–100% in some subgroups) and from 0% to 56.0% in the control group. Criteria for defining biofilms varied across studies.
Conclusion: There is currently no standardized reference method for detecting biofilms in chronic rhinosinusitis; existing evidence is derived primarily from patients undergoing surgical intervention and is limited by small sample sizes as well as heterogeneity in techniques and diagnostic criteria. Standardization is required regarding sampling sites, processing of pathological specimens, diagnostic criteria, and result reporting.
