DISTRIBUTION OF BETA-LACTAM RESISTANCE GENES IN KLEBSIELLA PNEUMONIAE AT NGUYEN TRI PHUONG HOSPITAL

Abstract

Objectives: To determine the prevalence and distribution of beta-lactam resistance genes (AmpC, ESBL, carbapenemase) in 388 K. pneumoniae strains isolated from inpatients at Nguyen Tri Phuong Hospital, and to evaluate the genotype–phenotype association. Methods: Cross-sectional descriptive study on 388 K. pneumoniae strains, March 2024–June 2025. Antimicrobial susceptibility testing followed CLSI M100-Ed35 (2025); genes were detected by multiplex real-time PCR. Results: ESBL genes were detected in 52.1%, carbapenemase in 42.5%, and AmpC in 16.2%. ESBL + carbapenemase co-existence was found in 32.5%; blaNDM + blaOXA-48 was the most common combination (19.6%). blaNDM, blaOXA-48 , and blaKPC were significantly associated with imipenem resistance (p < 0.00001). Genotypic ESBL (52.1%) and AmpC (16.2%) prevalence exceeded their phenotypic counterparts (28.9% and 5.9%). Conclusions: K. pneumoniae at this hospital carries a high burden of beta-lactam resistance genes, notably blaNDM + blaOXA-48 co-existence, with genotypic rates exceeding phenotypic detection, underscoring the complementary role of molecular testing in resistance surveillance.
