ANESTHETIC OUTCOMES OF LOW-FLOW DESFLURANE ANESTHESIA IN COMMON BILE DUCT STONE SURGERY AT QUANG NINH PROVINCIAL GENERAL HOSPITAL IN 2025

Abstract

Objective: To describe clinical characteristics affecting anesthetic effects and evaluate the outcomes of low-flow anesthesia using Desflurane in patients undergoing common bile duct (CBD) stone surgery.

Subjects and Methods: A study was conducted on 22 patients who underwent surgery for CBD stone removal at Quang Ninh Provincial General Hospital from March 2025 to September 2025.

Results: The mean operative time was 110.32 ± 22.14 minutes (range: 90–150 minutes). The mean anesthesia time was 122.45 ± 28.26 minutes (range: 100–180 minutes). The mean total intraoperative Fentanyl dose was 295.72 ± 98.35 µg (range: 150–450 µg). The mean recovery time was 8.72 ± 3.45 minutes (range: 3–20 minutes). Changes in Mean Arterial Pressure (MAP), heart rate, SpO2, and ETCO2 remained stable throughout the procedure compared to baseline (T0). No patients recorded a PRST score \geq 2; only 1 patient had a PRST score of 1 at time point T1. Immediately after extubation and 5 minutes post-extubation, 8 patients (36.36%) reported pain. All patients were able to accurately provide their name, age, and address upon awakening. The quality of anesthesia was rated as "Good" in 100% of cases. During the recovery phase, 10 patients (45.45%) experienced postoperative nausea and vomiting (PONV), and 2 patients (9.10%) experienced shivering.

Conclusion: Low-flow anesthesia with Desflurane maintains stable hemodynamics, provides high anesthetic efficacy, ensures rapid emergence, and offers high recovery quality.
