Objective: To establish age- and sex-specific serum creatinine reference intervals for healthy Vietnamese children.
Subjects and Methods: A cross-sectional study was conducted in 3,547 healthy children aged from birth to <19 years who were recruited from the community in seven provinces and cities across Vietnam between 2024 and 2026. Serum creatinine concentrations were measured using an enzymatic assay on the Roche cobas pro platform. Outliers were identified and excluded using the Horn method. The need for age and sex partitioning was evaluated using the Harris-Boyd method, the Mann-Whitney U test, and Cliff's delta. Reference intervals were established using the nonparametric approach in accordance with the CLSI EP28-A3c guideline.
Results: Serum creatinine concentrations varied markedly with age, being highest during the neonatal period, decreasing rapidly within the first two weeks after birth, and gradually increasing until adolescence. All predefined age boundaries met the criteria for partitioning, whereas sex-specific reference intervals were required only for the 15 to <19 years age group. Age- and sex-specific serum creatinine reference intervals were established for healthy Vietnamese children.
Conclusion: Age- and sex-specific serum creatinine reference intervals for healthy Vietnamese children were established in accordance with the CLSI EP28-A3c guideline. These reference intervals provide a basis for interpreting serum creatinine results in pediatric patients.
