ASSOCIATION BETWEEN THE HBA1C/HDL-C RATIO AND CARDIOMETABOLIC MULTIMORBIDITY AMONG ADULTS AGED 45 YEARS AND OLDER

Abstract

Objective: To evaluate the association between the HbA1c/HDL-C ratio and cardiovascular-metabolic multimorbidity (CMM) among patients aged 45 years and older at Phenikaa University Hospital.

Methods: A cross-sectional descriptive study was conducted on 1,502 patients aged 45 years and older at Phenikaa University Hospital from March 2026 to June 2026.

Results: The prevalence of CMM in the study sample was 29.7%. When the HbA1c/HDL-C ratio was analyzed as a continuous variable and divided into quartiles (Q1–Q4), the prevalence of CMM progressively increased from 13.3% in Q1 to 50.3% in Q4. In the multivariable model, the Q4 group had a 4.77-fold higher likelihood of having CMM compared to the Q1 group (aOR = 4.77; 95% CI: 3.25–7.01), with a p-trend < 0.001. When analyzed as a continuous variable, each unit increase in the HbA1c/HDL-C ratio was associated with a higher likelihood of CMM (aOR = 1.31; 95% CI: 1.21–1.41).

Conclusion: A high HbA1c/HDL-C ratio is independently associated with CMM in patients aged 45 years and older. The HbA1c/HDL-C ratio holds potential as a simple biomarker reflecting the burden of cardiovascular-metabolic diseases; however, prospective studies are needed to evaluate its predictive value.
